There are three transitions that need to be monitored. The first is from the hospital-based universal hearing-screening program to the diagnostic program. Hospital screening programs must be aware of the diagnostic audiologists in the area. The second transition occurs between the diagnostic audiologist and the early intervention program. The audiologist must have a system in place to assure a smooth referral process. The program conceptualizes the start of intervention to occur at the time of diagnosis. The third transition is from the early intervention program to the preschool program. Long before this transition is made, the school district must be aware that the child is in the community so that appropriate services are in place when the child starts preschool. The program has endorsed having the CHIP facilitator continue family-centered work during the transition process. This process should be completed, for most children, by the time the child is 3 1/2 years old. This philosophical commitment to continue family-centered intervention during this extended transition time is supported by HCP, which funds the facilitator during the transition process. In some cases payment is requested from the Part B program.
The transition from early intervention services into preschool services can often be a confusing time for children with disabilities and their families.
This transition occurs when a child turns three years old and the switch is made from using the Individualized Family Service Plan (IFSP) to using the individualized Educational Program (IEP).

As parents or guardians of a child with special needs you have the right to the following: · A transition planning process that begins at least 6 months before your child turns three · A written transition plan that is a part of your child's IFSP · A representative from the School District should be involved in the transition planning process.
