– Student Health Services
Student Health is committed to providing a safe and healthy environment for students. Student Health provides services on campus from 7:00 AM until 10:00 PM with a nurse on call after hours. Student Health administers or delegates medications, provides first aid and emergency care, collaborates with parents to develop health care plans, and provides health education to students and staff. Student Health adheres to regulations set forth by the Colorado Department of Education for school health.
• Current Immunization Records are Required.
• *If a student is to receive any medications at CSDB, other than the parental approved over-the-counter medication supplied by the school, a Medication Administration Authorization form (opens as a Word document) must be completed and signed by both the parent/guardian and a physician. This includes prescription and non-prescription medications. A student is not allowed to keep medication with them during the school day or in their dorm room that has not been approved by a student health nurse.
• *If your student requires a special diet for allergies or special food preparation (i.e. chopped, pureed) a Food Modification – Dietary Disability or Food – Meal Modification form must be completed and signed by your child’s physician, every year. For Preschool students, a Preschool – General Health Appraisal Form must be completed by a health care provider before your child may start preschool. A preschool physical is required annually.
If a middle school or high school student wishes to participate on a CSDB sports team, (the following is a link to) CHSAA sports physical must be completed, signed by parents/guardian and a doctor, and forwarded to the Athletic Director, prior to beginning practice. A sports physical will be required annually.
If a student wishes to participate in Special Olympics, (the following is a link to) Special Olympics Physical form must be completed every 3 years, signed by parents/guardians and a doctor. The completed form should be forwarded to Ms. Mimi McGinty at CSDB.
